I had torqued both outer mounts to the factory spec of 15 ft lbs. When going to bolt them to the lower bar, the one that attaches to the van body, the holes didnt line up. So I loosened them to make things work. When I did, I torqued the upper nuts to the studs and, the nuts started just spinning. Not the studs, just the nuts, so the threads inside must be just gone. Happened to both outer ones I received.
